How to set up Epic Haiku on your phone
- Install Epic Haiku from the App Store (iOS) or Google Play (Android).
- Below, find your hospital. Scan the Haiku setup QR code on top, or tap the Haiku setup link, to open that hospital's Haiku configuration page on your phone.
- Tap the configuration file the page offers — the Haiku app picks it up automatically.
- Open Haiku and sign in with your hospital-issued provider credentials.
- If multi-factor authentication prompts: complete it with the same method you use for that hospital's remote access (DUO, Symantec VIP, Microsoft Authenticator, etc.).
If the configuration URL doesn't work, or your hospital uses an enrollment form (RWJBarnabas and White Plains), scan the helpdesk QR code below it (or tap the helpdesk number) and ask for the Epic Haiku team.

Enrollment: providers must submit the White Plains Hospital Haiku Request Form (PDF available from the helpdesk) to ITServiceDesk@montefiore.org. The form requires: name, email, department, phone, role, device type (Apple / Android), and the Phone IMEI ID — found inside the Haiku app on the phone.

Only the helpdesk QR code is shown for these hospitals. Call them if you need to ask whether Epic Haiku is offered.
St. Peter's University Hospital (SPUH)
Epic Haiku: NOT AVAILABLE. SPUH does not provide mobile Epic access for outside physicians. Email is permitted only via Remote Citrix on a computer; direct outside-physician email is not allowed.
